A vial usually contains multiple doses. In STEP 1, participants began at 0.25 mg once weekly for 4 weeks. The protocol increased the dose every 4 weeks through 0.5 mg, 1.0 mg, and 1.7 mg to a 2.4 mg target at week 16; lower maintenance doses were permitted when 2.4 mg caused unacceptable side effects. This describes the study protocol, not a personal dosing recommendation.

How to reconstitute semaglutide

Reconstitution means adding a sterile diluent to a freeze-dried (lyophilized) powder. Bacteriostatic Water for Injection is a sterile multi-dose diluent containing 0.9% benzyl alcohol; use it only when it is appropriate for the contents of your vial. The diluent and storage instructions supplied with a specific product take priority.

Choose the diluent volume

Most peptide vials hold no more than 3 mL. If you do not have a different volume specified, 2 mL is a practical default: it fits most vials and usually produces a concentration that is straightforward to measure on an insulin syringe. Do not add more liquid than the vial can hold, and do not use tap, bottled, or other non-sterile water.

The amount of diluent changes the concentration, not the total amount of peptide in the vial. More liquid creates a lower concentration and a larger syringe draw for the same dose; less liquid creates a higher concentration and a smaller draw. Enter different water volumes in the calculator above before mixing to compare the resulting concentration and draw.

A vial may still look partly empty after reconstitution because the lyophilized powder occupies very little space. The dose is determined by the concentration you created—not by how full the vial looks.

  1. Prepare a clean work area. Wash and dry your hands. Gather the vial, the correct sterile diluent, alcohol swabs, new sterile syringes and needles, and a sharps container.
  2. Inspect the vial and diluent. Check the names, amounts, expiration dates, seals, and expected appearance. Do not use a container with a damaged seal or visible contamination.
  3. Record the planned volume. Use the calculator to confirm the concentration and draw volume, then write down the vial amount and exact diluent volume before you begin.
  4. Clean the vial stoppers. Wipe each rubber stopper with a fresh alcohol swab and let it dry completely. Avoid touching the cleaned surface.
  5. Draw the diluent. With a new sterile syringe and needle, draw the exact volume you selected. Remove large air bubbles so the transferred volume remains accurate.
  6. Add it slowly along the vial wall. Direct the stream down the inside glass rather than forcefully onto the powder. This reduces foaming and agitation.
  7. Gently swirl until dissolved. Rotate or roll the vial gently. Do not shake it. Let any foam settle, then inspect the solution for undissolved material, particles, cloudiness, or unexpected color.
  8. Label the finished vial. Record the peptide amount, diluent and volume, resulting concentration, mixing date, and the storage or discard date that applies to that formulation.

Storage and when to discard it

Storage temperature and usable life after mixing depend on the peptide, diluent, sterility, and formulation. Bacteriostatic water allows repeated withdrawals from its own container, but it does not create a universal 4- or 6-week stability period for everything mixed with it. Use the storage conditions and beyond-use date supplied for the product. Discard a mixed vial if that date passes or if the seal is damaged, the solution changes appearance, or contamination is possible.

How to draw from a vial

First confirm that the calculator result matches the syringe in your hand. On a U-100 syringe, 100 units equals 1 mL; other syringe types use different markings.

  1. Start with clean hands and a new sterile syringe and needle. Never reuse a needle or syringe and never share injection equipment.
  2. Clean the vial stopper with a fresh alcohol swab and let it dry.
  3. Draw air into the syringe equal to the liquid volume you plan to withdraw. Insert the needle through the stopper and push that air into the upright vial without forcing or over-pressurizing it.
  4. Invert the vial and keep the needle tip below the liquid surface.
  5. Pull back slowly to just past the target mark. Tap bubbles toward the needle and push the air and excess liquid back into the vial.
  6. Read the final volume at eye level. Align the correct edge of the plunger with the target line, confirm the units and volume one more time, and then remove the needle.
  7. Dispose of used sharps promptly. After one use, place the needle and syringe in an FDA-cleared sharps container. Do not leave loose needles in household trash or recycling.

How semaglutide maintenance doses are selected

Semaglutide product labels are indication- and presentation-specific. For Wegovy injection, the current FDA label starts at 0.25 mg once weekly and uses 4-week escalation steps. If a dose is not tolerated, the label says clinicians may consider delaying escalation for 4 weeks.

For adult weight reduction, the label lists 1.7 mg or 2.4 mg once weekly as maintenance doses, with 2.4 mg recommended. It directs clinicians to consider treatment response and tolerability when selecting between them.

The label reports an elimination half-life of approximately 1 week. Weekly doses therefore overlap in the body rather than acting as isolated events, which is why response and tolerability are evaluated over time instead of from one dose alone.

The current label also allows a clinician to consider 7.2 mg only after 2.4 mg has been tolerated for at least 4 weeks and additional weight reduction is clinically indicated. That is not an automatic endpoint or the top of a universal dose ladder.

Understanding the calculation

Concentration equals the amount in the vial divided by the liquid volume. For example, adding 2 mL to a 10 mg vial produces 5 mg/mL. A 2.5 mg dose at that concentration occupies 0.5 mL.

On a U-100 syringe, 1 mL is marked as 100 units, so 0.5 mL is 50 units. “Units” describe volume, not medication amount: the same unit mark can contain a different amount when the concentration changes.
